Request to Address City Council - Request must be received by the Monday prior to the posting of a meeting to appear on the next City Council meeting. Application for Board - Individuals who would be interested in serving on these boards and commissions

Conflict of Interest - All vendors and other persons contracting or seeking to contract for the sale or purchase of property, goods, or services with a local government entity must file a conflict of interest questionnaire with the City Secretary
Police Department
Police Applicant Registration - For those interested in employment with the Dickinson Police Department.
Business Contact Information - Businesses can keep the police department notified of emergency contact information.
Police Report Request Form - Request copies of police reports that have been previously made.
Vacation Watch - Request extra patrols when you are away from home.
Narcotics Activity Report - Report suspected Narcotics Activity.
Traffic Enforcement Complaint - Report traffic complaints.
Physical Assessment Release Form - Form to be completed prior to pre-employment physical agility test.

Driving Record Request - Available from the Texas Department of Public Safety web site. Driving Safety Course Affidavit - Defensive Driving Ticket Dismissal Form Time Payment or Extension Form - Application for a payment plan or time extension of fines.
Community DevelopmentInspection Request Form - Use this on line form to request a building inspection.
