The Personal History Statement should only be completed when instructed.  For police officer applicants, it should be completed by applicants who have successful completed both the written entrance exam and the physical agility test. 

 

The document can be downloaded to your computer by right clicking on the above link and selecting save target as or save link as, depending on your browser and operating system. The Personal History Statement can be either typed using Microsoft Word or Adobe Acrobat Reader or handwritten.  It should be printed and the final two question completed in the applicant's own handwriting. 

 

The completed Personal History Statement, along with supporting documents, should be submitted to the Dickinson Police Department at 4000 Liggio, Dickinson, Texas.

BEFORE PROCEEDING

 

These instructions are provided as a guide to assist you in properly completing your Personal History Statement. It is essential that the information be accurate in all respects. It will be used as the basis for conducting a background investigation that will determine your eligibility for employment. 

  1. Your Personal History Statement should be typed or printed legibly in black ink by you and no other person. Answer all questions to the best of you ability.

  2. If a question is not applicable to you, enter N/A in the space provided.

  3. Avoid errors by reading the directions carefully before making any entries on the form.  You may make copies of the form before you begin completing it.

  4. Be sure your information is correct and in proper sequence before you begin.

  5. You are responsible for obtaining correct names, addresses, telephone numbers, and zip codes. If you are not sure of an address, check it by personal verification. Your local library or the use of an Internet service may serve as a resource for information.

  6. Incomplete information may result in disqualification.

  7. If there is insufficient space on the form for you to include all information required, attach extra sheets to the Personal History Statement. Be sure to specify the relevant section and questions number before continuing your answer.

  8. An accurate and complete form will help expedite your investigation. On the other hand, deliberate omission(s) or falsification(s) may result in disqualification.

  9. Upon completing the form, re-check each section to ensure that all information requested have been provided, or N/A entered if appropriate.

  10. Attach copies of all certifications, licenses, diplomas, and legal documents, such as marriage license, divorce decrees, etc. to application.

  11. The final two questions must be completed in the applicants own handwriting in the space provided.
